The Pledge a Place Campaign is a fundraising scheme designed to sponsor a place for an individual on the Helen Rollason Live Well Experience. You or a group of people can join together to raise the £500 needed to sponsor a place.

If you would like to take part in this scheme, please complete and return this form. Upon receipt of the completed form, you will receive:

• The name of a member of staff to support your fundraising activities.
• Tips and Ideas for fundraising.
• Helen Rollason promotional material.
• Charity newsletter.
